Patient Forms

You can save time and expedite your first visit by completing our Online Patient Forms at home. Also, remember to bring your insurance card and an ID with you to your first appointment.

NOTICE TO PARENTS OR LEGAL GUARDIANS

Minors who are not accompanied by their parent/s or legal guardian must present a written consent letter signed, and authorizing the companion to bring the patient.

NOTE: THE COMPANION MUST BE OF LEGAL AGE.

The letter must include the following:

The complete name of companion
Validity of the consent letter (indicate if only for appointment date or multiple visits)
Attached copy of patent/s or legal guardian’s valid government issued ID (driver’s license, passport, etc.)
Signature with date

The companion must bring the following:

Written consent letter with signature of parent or legal guardian
Valid government issued ID with photo (driver’s license, passport, etc.)

What To Expect At Your First Visit

Your first visit to our office is designed to get you better acquainted with all we offer as well as introduce you to our caring staff.

On your first visit, we will review your medical and dental histories and evaluate your oral health. We will take digital x-rays of your teeth and explain to you whether or not you will require any dental treatment.
